Form 1095-C Availability Notice
TCOMM911 is required under federal law to prepare Form 1095-C, Employer-Provided Health Insurance Offer and Coverage, in accordance with the Affordable Care Act (ACA). Form 1095-C provides information about health insurance coverage that was offered to eligible employees during the calendar year.

How to Request a Copy of Form 1095-C
If you were not a full-time employee, or if you are a former employee or other individual who needs a copy of Form 1095-C, you may request one at no charge.
Requests must include:
- Full name
- Current mailing address
- Last four digits of Social Security number (for identity verification)
- Calendar year requested
- Contact phone number or email address (in case clarification is needed)

Form 1095-C provides information about health coverage offers and enrollment. It may assist you in preparing your federal income tax return, but is not required to file your taxes.
